Lakewood Community Health Needs Assessment
How healthy is Lakewood? We found out!
On September 9, 2022, we released the Lakewood Community Health Needs Assessment report examining the social determinants of health in the City of Lakewood.
On January 17, 2023 we released the accompanying Action Plan with ideas from residents and community leaders on opportunities that exist to make Lakewood even more livable.
Residents offered feedback on resources such as food, housing, economic and social relationships, transportation, education, and health care. Examining the social determinants of health will aid the development of equitable community programs and initiatives for Lakewood residents during this two-stage process.
The report was built on primary data from the US Census as well as secondary data tied to a broad, statistically representative community survey, numerous focus groups and interviews with key stakeholders in the City of Lakewood. The report is organized around the Center for Disease Control’s definitions for social determinants and documents key indicators of community health: economic stability, access to both quality education and health care, neighborhood and built environment, and social and community context.

More than 60% of your health is determined by where you live.
The health of a city is determined by the people who live, work, and play there. Identifying Lakewood’s strengths and challenges will play a critical role in developing, implementing, and improving health-centered community programs and initiatives. Community input will help highlight Lakewood’s unseen challenges and determine how residents can best be served.
The CHNA Assessment Report released in September 2022 named the needs; the CHNA Action Plan released in January 2023 offers a potential framework for more equitable programs and initiatives to serve all Lakewood residents.
