Lakewood Community Needs Health Assessment
How healthy is Lakewood? That’s the question the Lakewood CHNA is hoping to answer.
The Lakewood Community Health Needs Assessment will examine the social determinants of health in the City of Lakewood. Your feedback on resources such as food, housing, economic and social relationships, transportation, education, and health care will help us develop equitable community programs and initiatives for our residents.
All Lakewood residents have an equal opportunity to receive a survey, but not all residents will receive one to maintain our evidence-based research methodology. The survey is only one of several methods that the CHNA process will use to gather feedback from a broad group of stakeholders. We’re inviting the community to contribute through surveys, focus groups, and other events. Sign up for our emails and we’ll let you know when meetings and events are happening in your neighborhood.

The City of Lakewood, the Healthy Lakewood Foundation, and the Three Arches Foundation have selected The Center for Community Solutions (Community Solutions) as a consultant to perform a Community-focused Health Needs Assessment (CHNA) and Action Plan for Lakewood, Ohio.
Over the next few months and running through 2022, this unique and community-based participatory effort will first focus on a holistic evaluation of our community’s strengths and weaknesses in relation to the social determinants of health, where Community Solutions will perform deep engagement with Lakewood’s diverse community to generate critical information and resident perspectives. Data gathered through direct outreach will be augmented with secondary health data from other sources.
When complete, both the assessment and Action Plan are intended to play a critical role in developing, implementing, and/or improving health-centered, equitable community programs and initiatives that serve all Lakewood residents.

More than 60% of your health is determined by where you live.
The health of a city is determined by the people who live, work and play there, and we hope to learn about the health of Lakewood by examining social determinants of health. Resources such as food supply, housing, economic and social relationships, transportation, education, and health care all work together to determine length and quality of life. Identifying Lakewood’s strengths and challenges will play a critical role in developing, implementing, and improving health-centered community programs and initiatives. Community input will help highlight Lakewood’s unseen challenges and determine how residents can best be served.
Because we need to meet the needs of the entire community, this project will produce something comprehensive, actionable, and mutually supportive. By the end of 2022 we will be able to release a plan for more equitable programs and initiatives to serve all Lakewood residents.
